The Intermediate Care Service is a range of community based services providing Rehabilitation, Physiotherapy and Occupational Therapy. The service supports people who have been discharged from hospital to return to their previous or optimum level of independence. The service also provides nursing support to patients at end of life who wish to remain in their own home.
